CANCELLATION OR
MISSED APPOINTMENT POLICY
To continue to provide timely, quality care, the following policy has been created for the patients of 3 Rivers Health Centers:
1. Unless there are extenuating circumstances, a 24-hour notice of cancellation is required for all scheduled appointments. Failure to give a 24-hour notice may result in a $50.00 fee.
2. You will be asked to reschedule your appointment if you are more than 15 minutes late.

This policy applies to all products purchased from 3 Rivers Health Center or Blue Moon Apothecary, both Blue Moon Products, BioMatrix products, Herbal Plus, My Forte Oils and any other 3rd party products sold by us.
Within 30 Days of Purchase
Returns accepted on unopened products only
Full refund issued to original payment method
Return shipping is free only if the error was ours (wrong item or damaged in transit)
If the return is due to customer preference or ordering error, the customer is responsible for return shipping
Refunds are issued after the returned product is received and inspected
31–60 Days from Purchase
Returns accepted on unopened products only
Refund issued minus a 20% restocking fee
Customer is responsible for return shipping
Refunds are issued only after the returned product is received and inspected

No store credit
Product Condition Requirements
To qualify for a return:
Products must be unopened, unused, and in original packaging
Proof of purchase is required
Opened products are not eligible for return due to hygiene and safety standards
Damaged or Incorrect Orders
If your order arrives damaged or you received the wrong item, we will replace it at no cost to you. Please contact us promptly so we can resolve the issue.
